Understanding Medical Insurance Benefits
The Center for Health and Wellbeing (CHWB) is an integrative holistic medical practice with practitioners who address all aspects of a patient’s wellbeing. The foundation of this practice is based on the fact that we address the whole person; mind, body and spirit. Since we conceptualize our work through a holistic approach the referring from one practitioner to another is at the heart of our philosophy.
CHWB accepts most major medical insurance plans. The following information will help you understand which benefits are covered by what insurance. Of course, CHWB cannot guarantee coverage. This benefit is strictly between you and your insurance provider.

Medical Benefits Definitions
PPO – Preferred Provider Organization – Payment is received for each visit, treatment or procedure (Fee for service). We accepted most PPO plans.
HMO – Health maintenance organization – There are two basic types: Capitated HMO and Direct contract HMO. We do not accept any capitated HMOs. Direct Contract HMO is an HMO plan that also requires the patient to choose a PCP and get approval for all outside referrals. CHWB accepts Cigna, Sharp Health Plan and Tri-care as direct contract HMO polices.
POS – Point of service is a type of HMO benefit that allows the patient to access a medical provider out of their network by paying a given amount per visit. Therefore, any patient that wants to benefit from CHWB’s services may use their out of network option.
Co-pay- This fee is established between you and your insurance provider and must be paid to your medical provider at each visit.
Deductible- This fee is established between you and your insurance provider. If you have a deductible, the total amount must be paid before your insurance begins paying for your care. Most deductible style policies cover preventive care such as an annual exam before the deductible is satisfied. If you have a deductible, CHWB expects payment for your visit at the time the service is rendered. Lab tests, x-rays and medicines may all fall under the deductible. To save patients money, CHWB offers reduced rates on blood work done at our office. Any money spent out of pocket will go towards the patient’s deductible as long as the patient submits the receipt to their insurance company.
